Note: For players under 18 years of age, a parent or legal guardian must complete this form on behalf of the player.
Terms and Conditions
This Loan Agreement is made between: Club: Northern Beaches Heat Basketball Club (Trading as Cairns North Basketball Inc)

1. Loan Item
The Club agrees to loan the following item to the Player:
- Item: Basketball Singlet
- Singlet Number: As specified in the form above
- Season: Summer 2025/26
2. Purpose
The singlet is loaned for use in:
- Official team games
- Training sessions (if desired)
- Related team events during the Summer 2025/26 Season
3. Condition & Care
The singlet is being loaned in good, wearable condition. The Player agrees to:
- Take reasonable care of the singlet
- Not alter or damage the singlet
- Wash and maintain the singlet appropriately
- Return the singlet in good condition (excluding normal wear and tear)
4. Return of Singlet
Failure to return the singlet by the specified date may result in the Player being charged for its replacement.
5. Replacement Costs
Important: If the singlet is lost, stolen, or damaged beyond normal wear and tear:
- For players under 18: The parent/guardian agrees to pay the replacement fee
- For players 18 and over: The player agrees to pay the replacement fee
Payment will be required before the player can participate in future seasons.
6. Acknowledgment & Agreement
By completing and submitting this form:
- If the player is under 18 years of age, the parent/legal guardian acknowledges that they have read, understood, and agree to all terms and conditions on behalf of the player, and accept full responsibility for compliance with this agreement.
- If the player is 18 years or older, the player acknowledges that they have read, understood, and agree to all terms and conditions.
Your submission of this form constitutes your electronic signature and legally binding acceptance of these terms.
